Uterine Fibroids and Recurrent Early Miscarriage
Overview
Uterine fibroids are benign (non-cancerous) growths of the uterus. Uterine fibroids can range from mild to severe, depending on the fibroid(s) size, quantity, and location.
They can grow on:
- Perimetrium (outermost layer of the uterus)
- Myometrium (middle layer of the uterus)
- Endometrium (innermost layer of the uterus)
These fibroids will obstruct uterine blood flow, and during pregnancy, they will compete with the embryo for space, nutrients, and blood. For milder cases, it can lead to the baby being born smaller than normal. However, it is very common for early miscarriages as the fibroids quickly outcompete the embryo. The closer the fibroids are to the innermost layer of the uterus, the more dangerous it is for the embryo.
Note: Miscarriage and induced abortion may cause the development of Anti-embryonic Antibodies that will lead to recurrent early miscarriage.
Symptoms of Uterine Fibroids
About 90% of patients experience no signs of uterine fibroids. However, those with fibroids on the middle layer of the uterus may experience a heavy and prolonged menstrual period.
During the first trimester of pregnancy, the biggest tell-tale sign that a miscarriage is about to happen because of uterine fibroids is slow-rising hCG levels, despite normal progesterone levels. Patients will notice that their hCG level is not doubling every three days as expected.
If left untreated, patients can suffer from recurrent miscarriage.
Diagnosis of Uterine Fibroids
Antai hospital provides the following diagnostic tests on a case-by-case basis.
(Layer 2 hidden) Diagnostic Test 1: Panoramic 4D Color Doppler Ultrasound
It is an advanced B-Scan, with the added function of wide-angle, allowing for easy observation of uterine diseases. With ultrasound, doctors can directly spot uterine fibroids and check for their location and quantity.
Diagnostic Test 2: Blood Tests
With blood tests, doctors can check for estrogen levels and anemia.
Since estrogen stimulates the growth of fibroids, women with fibroids would have elevated estrogen levels. Due to the prolonged and heavy menstrual period some women experience, they may be anemic.
Treatment of Uterine Fibroids
Treatment of uterine fibroids does not require a hysterectomy (removal of the uterus), neither is it recommended.
Antai hospital provides the following fibroid removal surgery on a case-by-case basis.
- For women who are not pregnant:
The doctors at Antai hospital will remove the fibroids either via hysteroscopic surgery or 3D laparoscopic surgery, depending on their location.
3D laparoscopy:
- Fibroids on the outermost layer of the uterus
Hysteroscopy:
- Fibroids on the innermost layer of the uterus
3D laparoscopy or hysteroscopy, or both:
- Fibroids on the middle layer of the uterus
- Complicated case of fibroids on all layers
Above is a general guide. The ultimate decision depends on the case and is subjected to the doctor’s discretion.
2. For pregnant women
Antai hospital only recommends the removal of uterine fibroid if it poses a risk of miscarriage.
For pregnancies at stake, the doctors can only operate on patients with a small number of uterine fibroids on the outermost layer of the uterus via 3D laparoscopy.
Antai hospital has plenty of successful cases of removing uterine fibroids for pregnant women. However, there is still a certain level of risk imposed on the unborn baby, and not all pregnant patients are suitable for the surgery. Hence, the doctor in charge will have an in-depth discussion with the patients, and the ultimate decision of eligibility is subjected to the doctor’s discretion.
