Hyperemesis Gravidarum (HG) and its Treatment

Often dismissed, misdiagnosed, or not taken seriously, Hyperemesis Gravidarum (HG) is one of the most serious and rare forms of pregnancy complications a woman can possibly face during a pregnancy. HG most often is compared to its milder counterpart NVP (Nausea and Vomiting in Pregnancy), also commonly referred to as morning sickness. Morning sickness is considered a healthy and normal part of a pregnancy, that does not persist into the later trimesters, accompanied with very mild symptoms of light vomiting and nausea. However, HG is accompanied with similar symptoms, only much more violent, severe, and persistent, to the point that it drives to the brink of their sanity. NVP affects up to 70% of pregnant women, where as HG affects approximately 1-2% of all pregnancies.

Causes

Here at Antai Hospital, through years of research and investigation, our researchers have come to the scientific conclusion that the cause of HG amongst pregnant women has to be closely related to HCG but is not the direct cause of HG. We understand that there is something triggering and activating the Chemoreceptor Triggering Zone (CTZ) in the vomiting centre in such a way that it causes HG.

Our doctors have determined that these women have abnormally high amounts of these “HCG-Chemoreceptors” or they have hypersensitive HCG-Chemoreceptors compared to regular, normal women who do not develop HG.

Due to a combination of these 2 factors or one of the two, pregnant women develop HG when these free-flowing HCG molecules bind to these receptors in the vomiting centre, triggering a “Central Sensitisation” leading to the distinct HG symptoms that we know of including violent vomiting, extreme nausea, insomnia, excessive salivation etc.

Symptoms

Vomiting

You may experience extreme nausea and vomiting during your pregnancy which persists throughout the pregnancy

Insomnia

Insomnia is one of the common neurological symptoms caused by the HCG allergy. You may experience severe insomnia for up to several days, leading to emotional and mental instability.

Excessive salivation

Your salivary glands produce more saliva than average, leading to drooling. This is caused by the over-stimulation of the M3 receptors (Muscarinic acetylcholine receptor M3) in the salivary gland’s smooth muscles.

Other symptoms include:

  • Dehydration
  • Malnutrition
  • Pale complexion
  • Hyperthyroidism (T3, T4)
  • Weight loss during pregnancy
  • A decrease in urinary volume
  • Low blood pressure

Diagnosis

Beside a clear observation of patient’s symptoms, we will also conduct a standard blood test to measure the electrolyte levels in their body, severity of dehydration, malnutrition, and hormonal levels (specifically T3, T4). We will also check for anti-embryonic antibodies if you have a history of recurrent miscarriage.

It is very likely that patients who have experienced Hyperemesis previously will most likely develop it in their current or subsequent pregnancies.

Treatment

Desensitizer treatment

Treatment for Hyperthyroidism

  • Antithyroid medication such as methimazole may be administered to reduce the over-activity of the thyroid glands, but it is not a must.
  • Hyperthyroidism usually goes away once Hyperemesis is treated.

How does Hyperemesis cause recurrent immunological miscarriage?

Hyperemesis will cause malnutrition and Hyperthyroidism, which leads to corpus luteum insufficiency. This can cause miscarriage and potentially recurrent miscarriage during pregnancies.
