Operative Hysteroscopy for abortions is the safe approach

Operative hysteroscopy is a routine surgical procedure for the management of intrauterine disease in obstetrics and gynecology departments. The procedure will be performed by a gynecological surgeon, under general or local anesthesia, according to the standard practice of the center concerned, with the patient in the gynecological position. Antibiotics may be administered, according to the standard practice at Antai Hospital.  The cervix is grasped with pozzi forceps and dilated, with a Hegar dilator if necessary, to facilitate insertion of the hysteroscope. 

A hysteroscope is inserted through the vagina and into the uterus. Liquid will then be pumped into the uterus for expansion, and a light source on the hysteroscope will allow the surgeons to have a direct and clear view of the uterine cavity. The surgeons will use an Antai hospital’s specially designed grabber forceps to remove the embryo/fetus and all pregnancy tissues as a whole without breaking it into pieces. The use of energy is usually unnecessary and saline will be the preferred method. Before the operation, the appearance of the uterine cavity will be recorded for diagnostic purposes. The retained conception product will be resected from top to bottom with the surgical resector, without electric power, this being the method recognized as best protecting the endometrium in previous studies. The use of forceps or curettes to facilitate the removal of pregnancy tissues is permitted and will be closely monitored. An electrical current will be used only as a last resort, in cases in which the retained conception product cannot be removed without it. The distension medium will be measured and examined on entry and exit.

The most significant difference between hysteroscopy and D&C or vacuum aspiration is that hysteroscopy allows for direct visualization. The surgeons can directly look into your uterus on a screen with hysteroscopy and remove all pregnancy tissues with precision. Unlike hysteroscopy, surgeons performing traditional D&C or vacuum aspiration have to scrap your entire uterus lining to ensure that no remnants are left behind because they cannot directly see where the pregnancy tissues are.
