Diagnosis

To diagnose immunological recurrent miscarriage, a sample of your blood will be acquired for blood test and screening will be done via ELISA

  • If the levels of antinuclear antibodies in your blood is higher than 1:64, it is considered a positive result.
  • If the levels of antinuclear antibodies in your blood is lower than 1:64, it is considered a negative result.

The results of the blood test only serve as supplementary information, the final diagnosis still lies on the doctor. He/She will make an informed and accurate decision based on not only the blood test results, but also your symptoms and medical history.

Treatment

Antai Hospital’s immunotherapy works by inoculating a gene vaccine from the husband’s blood that can effectively counteract the anti-embryonic antibodies. The mechanism of action is by injecting filtered paternal cells carrying the identical chromosome 2 antigen, we can effectively negate the activity of the anti-embryonic antibodies, a form of immunity known as “immune-escape”. Although our immunotherapy does not eliminate the antibodies, it prevents the possibility of an immunological miscarriage and allows for a successful and healthy pregnancy to develop.

Step 1: Before treatment, the patient will be tested for presence of anti-embryonic antibodies, for 

              her hormonal levels, hysteroscopy test etc.

Step 2: Our doctors will acquire 160ml of venous blood from the patient’s husband, which will be    

             used to make the gene vaccine.

Step 3: The blood acquired will then be separated to its base components via centrifuge to isolate  

              the nuclear cells in the husband’s blood.

Step 4:  The acquired sample will be proliferated and processed in the laboratory using PCR. Cell    

              division will cause the chromosomes in the nuclear cell to replicate.

Step 5: We will now isolate the specific gene (chromosome 2) that is responsible for the encoding of 

              the fibronectin protein and inoculate it into a “vaccine”. This process takes about 4 days.

Step 6: Patients will now be administered with the gene vaccine via intramuscular injection. 

The entire course of treatment has 2 effective periods:

  1. 4 injections per week before getting pregnant.
  2. 4 injections per week after getting pregnant

*Please note that the patient must try for pregnancy as soon as the first 4 injections are administered within a 6-month period, the sooner the better.

The first vaccination will be done in the hospital, while the remaining 7 can be done at the comfort of your home. The vaccines are easy to administer and can be brought home. 

Antai Hospital will provide a vaccine carrier box with the frozen vaccines for you to bring home. While at home, the vaccines can be kept in freezing conditions in the refrigerator and has a shelf life of up to 2 years.

Special cases

  1. In case you are already pregnant and tested positive for the presence of anti-embryonic antibodies, you must undergo the immunotherapy as soon as possible to avoid a possible immunological miscarriage. Please discuss with our doctors upon consultation.
  2. If the husband is not present, no longer around or has a possible infection that renders his blood dangerous to use, Antai Hospital will opt for the use of cord blood, which is safe and effective in counter-acting the effects of anti-embryonic antibodies. Additional fees may apply.

How does immunological factor cause an early miscarriage?

Immunological factors cause an early miscarriage due to the formation and presence of a special type of antibodies, known as Anti-Embryonic Antibodies. Anti-Embryonic antibodies, as its namesake, targets embryonic cells, specifically these ANAs (anti-nuclear antibodies) target the chromosome 2 of the embryo during its rapid development in the early stages of pregnancy. It seeps pass the cell membrane of the embryonic cells and binds with the chromosome 2 as it is rapidly dividing during cell division, causing it to cease the production of the fibronectin layer.

This fibronectin layer that lines the microvilli of the embryonic cells serves as an immunological barrier between the maternal and embryonic cells. Without it, the embryo will be eliminated by the mother’s immune cells as they register the embryo as a foreign protein, causing what we define as an “immunological miscarriage”

How does a woman develop an immunological factor? 

Immunological factors arise due to the presence these anti-embryonic antibodies in the mother’s body during a pregnancy. During an induced abortion procedure, when demised fetus or embryo (and its attachments) is being removed from the mother’s uterus, there is a high risk of damaging the uterus lining via scraping. This exposes the uterine veins and allows these fetal cells carrying paternal genes to enter into the mother’s bloodstream. 

Once registered by the maternal immune system as an invading pathogen, her body develops these anti-embryonic antibodies as an immune response to attack and eliminate the fetal cells.
