Hyperemesis & Heartburn
Hyperemesis Gravidarum is considered a neurological disorder of the central nervous system, a consequence of the HCG-Chemoreceptors being hypersensitized in the CTZ (chemoreceptor trigger zone). A systemic central sensitization is triggered within the mother, continuously activating many muscarinic acetylcholine receptors M1 that are distributed in postganglionic nerves, common in several exocrine glands and the CNS. As we understand, these M1 receptors are involved in salivary secretions, gastric acid secretions, in several brain activities such as behaviors and memory, and several other effects.
This is why we observe Hyperemesis Gravidarum having several different kinds of symptoms of varying degrees depending on what the patient experiences. Although we do know its primary symptoms for ease diagnosis: Insomnia, Excessive Salivation, Violent Vomiting, and Nausea, but it carries several other lesser-known ones, which we call secondary symptoms.
In this article, we discuss one uncommon symptom: which is heartburn. This form of hyperemesis often replaces heartburn with the vomiting that we so commonly observed. So instead of the violent vomiting, these patients experience what is known as pregnancy heartburn accompanied by all the other pregnancy symptoms. Sometimes the heartburn itself can lead to vomiting and/or diarrhea. We believe that the underlying mechanism is also due to the m1 receptors of the stomach being hypersensitized first, triggering the backflow of stomach acid into the esophagus. This is then followed by symptoms of chest pain, the feeling of vomit, and slight nausea.
As it develops, nausea gets worse, vomiting gets violent, and you will probably experience some level of sleep deprivation. Eventually, it develops into Hyperemesis Gravidarum as we observe a loss in maternal body weight, dehydration, malnutrition, and electrolyte imbalances. This form of Hyperemesis has only been observed in one occasion so far, where in our history of over 10 years of dealing with Hyperemesis Management in Antai Hospital, have we encountered this symptom of Heartburn.
