What is Hyperemesis Gravidarum?

Complication of Hyperemesis Gravidarum – Recurrent Miscarriage

There are two main factors that can lead to an early miscarriage – uterine contractions and improper implantation. Hyperemesis gravidarum can cause both factors.

  • Uterine Contractions

Hyperemesis Gravidarum (HG) can lead to miscarriage because it involves frequent severe vomiting. As the uterus is made of smooth muscles, the uterus will contract each time you are vomiting. The frequent contraction of the uterus can potentially cause the embryo to be dislodged from the basal layer of the endometrium due to the unstable environment of the uterus. This will then lead to a miscarriage and even recurrent miscarriage if the HG is left untreated. 

  • Improper Implantation (Low Progesterone)

Severe vomiting due to hyperemesis gravidarum can also lead to undernutrition and eventually miscarriage due improper implantation caused by low progesterone levels. 

Low progesterone is a complication of HG that can lead to miscarriage. Patients with HG will basically vomit out everything they consume. When there is no food left in the stomach, they might vomit bile, blood, stomach acid or even the stomach lining.

As a result, patients will suffer from undernutrition, affecting the ability of the corpus luteum to produce progesterone. The progesterone is key to maintaining the pregnancy in the first trimester as it promotes the development of the chorionic villi. The chorionic villi are a hair-like substances that forms the outer layer of placenta and the “hair” acts like tree roots that burrows into the basal layer of the endometrium to ensure proper and secure implantation of the embryo. Secure implantation will reduce the sensitivity of uterine muscles and supress contractions, providing a stable environment for the embryo to grow. 

Hence, undernutrition resulting from of severe vomiting affects the ability of the corpus luteum to produce sufficient progesterone. Low progesterone levels will in turn hinder the development of chorionic villi and lead to insecure and improper implantation of the embryo. With improper implantation of the embryo, the embryo will be dislodged from basal layer of the endometrium easily and a miscarriage will occur. Patients will suffer from recurrent miscarriage if hyperemesis is left untreated.

  • Immunological Factor – Immunological RSA  (anti-embryonic antibodies)

Reproductive immunity disorder caused by anti-embryonic antibodies is not a direct consequence of hyperemesis gravidarum. It is, however, a consequence of recurrent miscarriage. Recurrent miscarriage is a very prominent occurrence in patients with hyperemesis gravidarum and many of them end up with reproductive immunity disorder caused by anti-embryonic antibodies on top of hyperemesis gravidarum.

Symptoms of Hyperemesis Gravidarum
