Antai Cervical Ring
Antai Cervical Ring is a proprietary cervical ring device used in a surgery, secured on the cervical neck, to prevent late miscarriages by enhancing cervical function.
Note: uterus anomalies can result in late miscarriages but cannot be treated with antai cervical ring because it will potentially rupture your womb due to lack of space.
Indication
- Cervical incompetence
- Congenital:
- The cervical has more muscles than connective tissues (1:1 or 1:2). As a result, there will be a lot of elasticity in the cervix, easy to open during pregnancy.
- People with congenital cervical incompetency: their cervical muscle ratio is too high. Their cervix muscle to connective tissues is the same as the womb. Womb is largely made of smooth muscles so that it can expand and accommodate a growing baby!!! Womb is originally 5 milliliters, when the baby reaches full term it can accommodate 5000 milliliters.
- Acquired:
- Birth trauma (cervix damage after childbirth)
- Short Cervix (opposite from infantile uterus)
- Congenital:
- Ratio of cervix is far smaller than uterus.
- Normal cervix: 3.5-4.5 cm
- Short cervix: < 3.5 cm
- Acquired:
- Conization: A procedure done to cut off part of a cervix to prevent cervical cancer.
- Conization is a surgery that removes the abnormal, precancerous cells from the cervix.
- As a result, the cervix will be too short.
Short cervix is under the big umbrella of cervical incompetency/insufficiency.

- During Pregnacy:B-Scan (check for dilation, length of cervix)
Method
- Via laparoscopy (transabdominal)
- Via vagina (transvaginal)
- Via laparoscopy + vagina (this method is only available at Antai hospital).
Preventive Surgery
Pre-pregnancy: Laparoscopy
- Preventive measure against late miscarriage.
Early Pregnancy: Laparoscopy
- Pregnant, but no symptoms of miscarriage yet. Internal orifice not opened yet. Usually, such patients have a history of recurrent miscarriage.
- Through the abdomen because the internal orifice has not opened yet.
- This is also a preventive surgery.

Emergency Surgery
Late Pregnancy: Either laparoscopy or Vaginal
- Threatened miscarriage (will turn inevitable without the surgery).
- Already in the second trimester.
- Internal orifice already opened, but external orifice not opened yet (can be spotted with B-scan).
- During pregnancy, our pelvic floor will be more relaxed because there is a lot of estrogen. This is to prepare for vaginal delivery.
- Hence, by this time, the vagina and pelvic floor are wider and more relaxed, allowing for transvaginal surgery. We can pull out the cervix and secure the Antai cervical ring.
- However, transvaginal surgery may not be as secure because may not be able to secure on internal orifice.

- Patients also require labor suppressant (Atosiban) to prevent premature labor and antibiotics Antibiotics to prevent infections because easy to have intrauterine infection (宫内感染).
Acute Surgery

Short Cervix
- Can only be a preventive surgery via laparoscopy, done pre-pregnancy.

Note: Can bed rest and lifting of hips/legs (trendenlenburg position) prevent late miscarriage?
