Uterine Fibroids and Recurrent Late Miscarriages

Overview

Uterine fibroids are benign growths of smooth muscles in the uterus, which is considered one of the factors causing recurrent late miscarriages. They account for 2 factors that cause miscarriages to happen:

  • Blood-Insufficiency / Competition of Nutrients (Early miscarriage)
  • Pressure Induced (Late Miscarriage)

Although they are benign, depending on their size and numbers, these fibroids will absorb nutrients and blood-flow in the uterus for its own growth. During early pregnancy, this is especially dangerous as it competes growth space, nutrients, and blood flow with the fetus. For massive cysts that are found during the late stages of pregnancy, they exert significant pressure on the pelvic and uterine cavity, in addition to the pressure of the already expanded and enlarge uterus that is hosting the fetus. All of this combined equates to an immense pressure build-up, and if not alleviated, can cause preterm birth and late miscarriages.

Fibroids can come in many different sizes, and they can appear as one large fibroid or multiple fibroids. Many women may have uterine fibroids and not know it because they often have no symptoms.

There are three types of uterine fibroids:  

  1. Intramural fibroids (grows within the uterine walls)
  2. Submucosal fibroids (grows and protrudes into the uterine cavity)
  3. Subserosal fibroids (grows and protrudes towards outside of the cavity, i.e., abdominal cavity)

Note: During a pregnancy, if the mother is diagnosed with uterine fibroid/s and it is determined to be endangering the pregnancy, Antai Hospital provides a removal surgery that can be conducted during pregnancy to save the fetus, provided that the fibroid is a subserosal fibroids (growing on the outer layer of the uterus). Other types of uterine fibroids cannot be removed via surgery and can only be done post-pregnancy.

Symptoms

Cysts usually are asymptomatic and sometimes even go away on their own. However, if you have a large ovarian cyst, you may experience:

  • Pain and swelling in the pelvic and lower back region
  • Bloating in the abdomen region

If you experience any pain in these regions, especially during a pregnancy, you should immediately seek medical attention.

Diagnosis

Most patients who have this complex disease are asymptomatic. A small portion experiences heavy menses with an extended menstrual period. To accurately diagnose the condition of fibroid-immunocomplex recurrent miscarriage, the patient must have had a history of early or late recurrent miscarriages. 

In general, these are the signs we look out for:

  1. Have a history of early and late recurrent miscarriage
  2. Experience heavy menses
  3. Abnormally long mense period
  4. Unable to get pregnant

The doctors will do an ultrasonography to determine the presence of fibroids in your uterus and to observe your uterine shape and size, followed by testing of your Estrogen levels and the presence of anti-embryonic antibodies in your bloodstream.


Treatment

  1. Before Pregnancy:

  1. During Pregnancy:

Treatment Method:

  1. Antai Hospital makes a precise incision on the abdomen of the patient and use our specialized 3D- Laparoscopy to surgically cut out the uterine fibroid.
  2. The abdomen cavity will not be filled with CO2 gas due to the already immense pressure in the cavity.
  3. To assist with exposure and retraction, Antai Hospital will suspend the patient’s abdominal pannus with a surgical retractor.
  4. The surgery will then proceed by removing the fibroids and after that, repair the incision opening.
