Fetal Demise or Stillbirth: Cause & Treatment – Antai Hospital

Fetal Demise: Cause and Risks – Antai Hospital

Fetal demise, also known as intrauterine fetal death, is a fetal arrest or death of a baby in the uterus. This situation is also referred to as stillbirth.  For stillbirths that occur within the first trimester, they are defined as embryonic demise. For those that are experiencing a stillbirth after week 12 and before 2o or 28, we call it a fetal demise.

This form of pregnancy loss results in the fetus with no signs of life. This is a form of diseases that can be treated according to its cause, which can be an array of different individual causes, approximately up to 43.

Signs & Symptoms

Generally, when fetal demise occurs, pregnancy reactions gradually begin to cease until they disappear completely. For example, the morning sickness, nausea, vomiting is the first to cease, breast sensitivity begins to drop as well. After that it will be followed by vaginal bleeding, often of a dark red color. Finally, lower abdominal pains and pressure begin to develop until the deceased fetus is expelled along with the pregnancy tissues. This, however, may not apply to all who are experiencing a stillbirth, some may experience no signs or symptoms at all. For these cases, only a ultrasound examination is able to confirm the fetal demise.

Cause

There are many individual causes that can lead to the same outcome of fetal demise, generally we observe that they are caused by:

  • Immunological Factor- Anti- Embryonic Antibodies (67%)
  • Uterine Factor
  • Ovarian Factor
  • Endocrine Imbalance
  • Genetic factors
  • Other causes (<3%)

Examination of a fetal demise

Here at Antai Hospital, we have a philosophy and recommendation for patients experiencing a fetal demise. Once confirmed with diagnosis of a stillbirth, instead of directly going into a D&C procedure to remove the remaining pregnancy tissue, we strongly urge you to take this opportunity to conduct a pre-operative vaginal hysteroscopic removal of the fetus and proceed with further analysis of the sample, to identify presence of fetal chromosome abnormality and pathology. With 4D- color doppler ultrasound, electrochemiluminsence, checking of anti-embryonic antibodies, inspection of uterine morphology and implantation site, laparoscopic inspection, we can have a better understanding of what is the cause that triggered this outcome.

As a rule of thumb, if you have a history of chemical pregnancy, fetal demise, or recurrent miscarriage, it is always better to seek diagnosis and treatment before going for your next pregnancy. Otherwise, you may risk another potential pregnancy loss, which may complicate the underlying causes and wastes more time.
